Hopewell Home Healthcare provides service to infants with complex medical needs to older adults needing assistance with Activities of Daily Living and everything in-between.

Our Staff includes Registered Nurses (RN), Licensed Practical Nurses (LPN), and Home Health Aides (HHA) whose focus is the one-on-one support of our clients and their needs. Hopewell Home Healthcare has the ability to provide nursing or aide support for any of our client’s needs.

Hopewell Home Healthcare provides comprehensive case management for every client. If you or a loved one need long term support, our experienced caregivers are available to provide compassionate and respectful care right in your own home.

01. SKILLED NURSING

These services range from teaching on new diagnosis or procedures to wound care and IV therapy in the comfort of your own home.

02. PRIVATE DUTY NURSING

RN or LPN/LVN who are working one-on-one with individual clients in the client’s home. Private duty nurses provide a higher level of care while working in tandem with the caregiver who is providing companion-homemaker services along with personal care services.

03. PERSONAL ASSISTANTS

Personal assistants spend a predetermined amount of time in your home assisting with activities of daily living such as bathing, grooming, cooking, light housekeeping, laundry, shopping and escorting to physician appointments.

04. HOMEMAKER

Home makers provide no direct patient care but do assist with cooking, cleaning, laundry and escorting high-risk clients on errands.

05. PEDIATRIC COMPREHENSIVE CARE

Our long-term skilled nurses care for chronically fragile children and are trained to manage the technologically advanced equipment they may need to manage respiratory and feeding needs and remain safely stable at home.

06. MEDICALLY DEPENDENT CHILDREN PROGRAM

This program is immediately available and provided to children from birth to age 21. It is designed to provide Comprehensive, private duty nursing to children with special needs. A chronic, lifelong illness is not required to participate. Some children may have severe, acute health issues but will eventually regain health and independence and can be weaned off services.

07. ALZHEIMER’S / DEMENTIA CARE

Alzheimer’s disease and dementia care is dedicated to preserving the happiness and independence of seniors living in their own homes for as long as safely possible. Our trained, dedicated team of care providers are ready to meet new seniors, and currently offers personal and dementia care in the homes of many seniors across eastern Texas.

08. COMPREHENSIVE CASE MANAGEMENT

Our highly screened and trained case managers deliver exceptional service while working closely with the family to develop an environment which nourishes the physical, mental and emotional well being of each client. We assess the unique medical and personal needs of each client and create a plan that ensures safety and wellness while maintaining maximal independence and privacy.
